A hospice nurse calls the family straight away and schedules the evaluation, usually the same day.

You stay informed
You keep your role as attending physician, you approve the plan of care, and you receive updates as the picture changes.
Eligibility at a glance.
Two physicians certify a prognosis of six months or less should the illness follow its expected course. Beyond that, the clinical indicators we see most often:
- Declining functional status — PPS or Karnofsky at or below 70 percent
- Unintentional weight loss, low albumin, or declining oral intake
- Recurrent hospitalizations or emergency department visits
- Dyspnea at rest, or increasing oxygen requirement
- Recurrent infections, including aspiration pneumonia and urinary tract infections
- Stage 3 or 4 pressure ulcers
- Increasing dependence in activities of daily living
- Treatment burden outweighing benefit, or a patient declining further curative treatment
If you are unsure whether a patient qualifies, call. We will review it with you, and we will tell you honestly if it is too early.
